Intent to Return 2021-2022

Instructions: Please complete the information below to inform Dailey Charter of your enrollment for the next year, no later than February 5, 2021, in order to hold your child's placement. Forms not received by the deadline will be dropped from enrollment for the 2021-2022 school year. If you have multiple students attending Dailey, each student needs a form completed.

I plan on submitting an application for a sibling (applications must be received during the open enrollment period): If yes, please include the grade level. *

Please indicate if your child is returning to Dailey. If your child is not returning, please provide the school they will be attending and the reason for leaving Dailey.

I believe in the IB Philosophy and gladly endorse the program policies and requirements. As a school of choice, we are happy to elect another year of a successful and academically challenging program.
Reason(s) for not returning:

Dailey Elementary Charter School Policies: Please read and type your name in each section verifying that you understand, accept, and will adhere to Dailey Charter's policies for continued enrollment. Dailey Elementary Charter is a school of choice in which you are agreeing to follow their policies and procedures for enrollment.

Parents/Guardians understand that Dailey provides a challenging and academically rigorous IB program that moves at an accelerated pace for students ready to work at grade level and above. *
Parents/Guardians will actively support their student's education and school community by attending parent conferences, school events, and respectfully communicating with staff. *
Parents/Guardians must inform the school of address and contact information changes and/or change in ATLAS. The school must be able to reach a parent/guardian during school hours. *
Students will be held to strict behavioral standards. Any behavior in or out of the classroom that is disruptive to the learning environment or school culture will not be tolerated. *
Parents/Guardians will hold the responsibility of reinforcing Dailey’s high academic and behavioral standards. *
I understand Dailey Charter actively protects instructional minutes and will follow all procedures and policies that support student learning. *
I understand that Dailey Charter is an independent charter school and not a Fresno Unified School. *
Students will follow the current Board Approved Dress Code policy every day without exception. *
Students will arrive at Dailey Charter by 7:40 a.m. every day prepared and ready to learn. *
Students will be picked up from Dailey Charter on time at the end of every day. • Kindergarten: 1:30 p.m. • 1st—5th 3:30 p.m. *
Parents and students will follow all other policies and procedures as outlined in the Parent/Student Handbook. *
